If you need a certificate, please come directly to the university's Executive Office office (Student Affairs Department Educational Affairs Divisiton) or request it by mail. Requests by phone, facsimile, or e-mail are not allowed.

What to bring at the time of application
- In the case of the person's request
- ID card (driver's license, health insurance card, etc.)
- Issuance fee (300 yen per copy.) Please be prepared so that you do not change.

- Power of Attorney (signature and seal of the person is required)

1. Certificate grant application form
Please download the certificate application form from the following.
- Certificate grant application form (graduation certificate, transcript, certificate of completion) 106KB
- Application for Certificate of Academic Ability (for obtaining an educational staff license) 116KB
If you can not print, please make a memo (application form) with the following items on A4 paper.
- Name, reading, and gender (in the case of an English certificate, write the name in Roman letters)
- Old name (only if the name is different from the name at the time of graduation or leaving the company)
- Date of Birth
- Undergraduate/Department
- Student ID number (if you do not know, you do not need to fill in)
- Date of enrollment and graduation
- Address of the mailing address
- Phone number (numbers that can be contacted during the day, such as home, mobile phone, workplace, etc.)
- The name of the required certificate and how many copies are required (in the case of an English certificate, please indicate so.)
- Strict seal required
2. A copy of your ID card (driver's license, health insurance card, etc.)
- Please enclose a copy of a certificate issued by a public organization that allows you to confirm your name, address, date of birth, etc.
- If the address has been changed and the new address is listed on the back, a copy of the back is also required.
- A copy of your ID card submitted will be used only for identity verification at the time of issuance of the certificate, and will be disposed of appropriately after confirmation.
3. Reply envelope
※The postage will be revised from October 1, 2024.
If the reply is likely to be after October 1, please attach the revised postage stamp.
- Please specify your home address, postal code and name on the reply envelope, and attach a stamp for the required amount. ※It cannot be sent to an address other than the person. (No work place, etc.)
- The type of envelope and postage vary depending on the type and number of certificates requested. The following postal charges are as a guide without sealing.
- If you are in a hurry, please attach an additional stamp for the express delivery fee. (Please attach a stamp for the regular mail charge + express delivery fee.) Only stamps for express delivery fees are not treated as express delivery.)
- If the postage is insufficient, you will need to send the missing stamp again.
- If the application cannot be accepted due to inadequate documents, etc., it will be returned in a reply envelope.

4. Issuance fees
- Fixed amount exchange of 300 yen per copy (eg, 900 yen if you apply for one graduation certificate and two transcripts)
In the case of mailing, the payment method is limited to fixed-rate small exchange rate.

5. Additional required documents for proxy application

- It may take up to one week for an English certificate. Please note that if there are any deficiencies or confirmation items by mail, the issuance will be further delayed.
Strict sealing process
Since it is issued on tamper-proof paper (except some certificates), it will not be strictly sealed. If the submission address has specified a strict seal, please fill out the application form to that effect.
- Strict envelope means enclosing the certificate in an envelope specified by the university and invalidating the opening. You will not be able to check the contents on your own.
About the certificate name
The certificate will be issued in the name of the student at the time of graduation or leaving the country. If you have changed your name after graduation, please prove it to your submission address.
