A medical checkup (fall semester) will be conducted as follows, so be sure to consult the target person.

1.Targets

(1)Students who were absent from school during the spring semester of 2017
(2)Students who had studied abroad in the spring semester of 2017
(3)Due to unavoidable circumstances, such as the person's illness,
　Students who were unable to receive a medical examination in the spring semester of 2017

2.Date and time of implementation

October 2 (Mon) to 4 (Wed) 13:30 to 16:30　
　　※We will not accept anything except this time.

3.Place of implementation

Shimonoseki City University Health Consultation Office
※If you are unable to receive a medical examination on this schedule, go to the hospital by yourself and receive a medical examination.
　Please submit a medical certificate to the Student Support Group.
　The cost of the consultation will be paid by you.

[Consultation Items]

　Height and weight ... for all
　Sight and blood pressure: For 4th grade undergraduate students and students who are attending teaching courses
　Diabetes, X-rays ... for all
　Electrocardiogram: Only for first-year undergraduate students

After all measurements are completed, please return your personal card to the University Health Consultation Office.
