The following medical examinations (autumn semester) will be conducted as follows, so be sure to take a medical examination.

1.Targets

(1)Students who were absent from school during the spring semester of 2017
(2)Students who studied abroad in the spring semester of 2017
(3)Due to unavoidable circumstances, such as illness of the individual
　Students who were unable to take a medical examination in the spring semester of 2017

2.Date and time of implementation

October 2 (Mon)-4 (Wed) 13: 30-16: 30　
　　※We will not accept any requests other than this time.

3.Implementation site

Shimonoseki City University Health Consultation Office
※If you are unable to have a medical examination on this schedule, you will go to the hospital on your own and have a medical examination.
　Please submit a medical certificate to the Student Support Group.
　You will be responsible for the consultation fee at that time.

[Consultation Items]

　Height and weight: For all employees
　Eyesight and blood pressure: For fourth-year undergraduate students and students who are taking teaching courses
　Urinalysis, x-ray ... For all members
　Electrocardiogram: Only for first-year undergraduate students

After all measurements have been completed, please return your personal card to the OIST Health Consultation Office.
