A medical examination will be conducted as follows, so be sure to consult the target person. 

1.Target person

(1)Students who were absent from school during the spring semester of 2015
(2)Students who studied abroad in the spring semester of 2015
(3)Students who were unable to undergo regular medical examinations in fiscal 2015 due to unavoidable circumstances such as their own illness 

2.Date and time of implementation

October 5 (Mon)-7 (Wed)
13:00～16:00　※We will not accept any requests except during this time.

3.Implementation site

Shimonoseki City University Health Consultation Office
※Students who cannot receive a medical examination on this schedule should go to the hospital at their own expense (self-pay), undergo a medical examination, and submit a medical certificate to the Health Consultation Office.

(1) Health Counseling Room
Height and weight: For all members
Sight and blood pressure: For 4th grade students and students taking teaching courses.

After the measurement is completed, bring your personal card to the Medical Association Hospital. 

(2) Medical Association Hospital
Urinalysis, X-ray ... everyone
ECG: 1st grade only 

When all measurements are completed, please return the personal card to the University Health Consultation Office.
